Who is eligible
If you are not sure whether someone qualifies, call us. We would rather spend ten minutes on the phone than have you guess.
- Adults 18 and older
- An I/DD eligibility determination through your county CDDP
- An ODDS-funded residential service authorization
- Oregon residency — we serve Clackamas and Jefferson counties
How a placement happens
Four steps, and the third one is the important one
Step 1
You call or send a referral
Tell us about the person: what they need, what they like, what has not worked before. We would rather hear the hard parts up front.
Step 2
We meet them
We visit wherever they are now. If it looks like a fit, they visit the house and meet the people already living there.
Step 3
The house agrees
The people who live in the home have a say in who moves in. This step is not a formality, and it is why our placements hold.
Step 4
Transition plan
We build the ISP supports with you and the person's team, set a move date, and overlap staff so day one is not a cold start.
